All new patients will be asked to complete a Medical History Form. This is so we know any relevant medical information that may affect the success of your dental treatment, and so we can deliver your treatment safely. All information you provide us with is strictly confidential.

You can download this form, complete and bring with you to your appointment. Alternatively, please arrive 10 minutes prior to your appointment and complete the form on reception.
